These are: the Personal Health Information Protection Act, 2004 (PHIPA), Freedom of Information and Protection of Privacy Act, 1990 (FIPPA) and the Municipal Freedom of Information and Protection of Privacy Act, 1990 (MFIPPA). > Receive an overview of what’s required through annual and point-in-time privacy breach reporting to the IPC. > Find out more about the application of administrative monetary penalties (APMs) available to the IPC as part of its enforcement powers for violations of the PHIPA and understand your responsibilities regarding access and privacy.

Rami Shami has been serving within Hospice Palliative Care for over three decades.He feels privileged to practice as a Consultant in Hospice Palliative Care, as well as Case Manager for the Second Mile Club at Kensington Health, and the Community Relations Advisor for Lighthouse for Grieving Children. While harvesting a conviction that everyone should have access to quality Hospice Palliative Care, Rami strongly advocates and supports the development and growth of programs and services for those travelling the journey of a life-limiting illness, their caregivers, and those who are bereaved. 

 

Ingrid Loepp Thiessen is the Chaplain at Sunnyside Home. In this role she provides spiritual care through home-wide discussion groups, faith circles, religious services and one to one support. She holds a Master of Divinity degree and is ordained for ministry. Ingrid has worked as a spiritual care provider in Long Term Care for over 15 years and is grateful for the chance to spend time with people in the final years of their lives and deeply honoured to be at the bedside of those who are dying. At Sunnyside she co-chairs the Palliative Care Committee and is chair of the Ethics Consultation Team. She loves to sing and finds ways to include music in her programming. She is married to Jim and they have three adult children.

 

Julie Abraham is the Acting Assistant Manager of care for Sunnyside Home. In this role, Julie is responsible for managing, coordinating, delivering & leading high-quality care as part of the care team using a person-centred approach on three home areas. Julie holds degree of Bachelor of Science-Nursing and Master of Science -Nursing from India. She also holds, Post Graduate certificate in Critical Care Nursing and Gerontology from Conestoga College Kitchener. Julie has worked as a Team Leader at Sunnyside Home and currently co-chairing the palliative committee.  Julie is passionate about the palliative care and enjoys helping residents with chronic illnesses, improving overall quality of life ,supporting and educating families.

 

Marie Bélanger is presently the Acting Administrator for Sunnyside Home but normally holds the role of Manager, Resident Care (Director, Nursing and Personal Care). In this role, Marie is responsible for leading, planning and the delivery of person-centered care for the 263 residents living at Sunnyside Home Long-Term Care. Marie started her nursing career in Québec, working at the Centre Hospitalier de l'Université de Montréal, Pavillon Notre-Dame.  She has been working within the Long-Term Care sector since 2002 and has held different roles such as Nurse Educator, RAI/MDS Coordinator, Director of Care and Resident Care Coordinator. She has a diploma in Nursing Studies from Québec, and she graduated from McMaster University with a Bachelor of Science in Nursing and an Oncology Certificate. Through her studies, she had the opportunity to do a nursing placement in oncology and palliative care in Switzerland.

 

Fred Mather, MD CCFP FCFP Dr. Mather, a former President of OLTCC, is the Medical Director of Sunnyside Home, the municipal home for the Region of Waterloo, and attending physician for an Extendicare home. Dr. Mather consults for the interdisciplinary primary care Memory Clinics. He currently serves as Vice Chair for the Ontario Association of Resident Councils.

 

Heather Toll RD is a Registered Dietitian with the College of Dietitians of Ontario.  Heather completed her Bachelor of Applied Science in Food and Nutrition at Toronto’s Metropolitan University in 2004. With over 19 years of experience as a Registered Dietitian practicing within Long-Term Care, Heather has a passion for geriatric nutrition and working with residents to enhance food enjoyment, assist with intake challenges and help residents achieve their goals of care.  Heather has been a key stakeholder in the development of an interdisciplinary palliative and end of life care program at Hillsdale Terraces long-term care home and is also a member of the Ontario Seniors Nutrition and Advocacy Committee (OSNAC). Heather strives to educate and empower long-term care registered dietitians to take an active role in palliative and end-of-life care programs as a key team member to assess and assist with early identification of health status decline.

 

Carol Donovan RD, President Seasons Care Dietitian Network Inc. Above all, Carol identifies as a foodie. Her hunger for combining excellent dining and hospitality experiences with her clinical nutritional expertise make Carol a unique, dynamic and innovative leader in the industry. As a Registered Dietitian for over 35 years, Carol's passion for seniors’ nutrition and her food first philosophy have put her at the forefront of her practice. It was her innovation and vision that led her to establish Seasons Care Dietitian Network 24 years ago. Today, Seasons Care is Canada’s Largest Dietitian Company and takes pride in putting Food First. The company continues to grow and excel under Carol’s leadership. Carol is an industry leader who promotes advocacy for seniors and dietitians alike. Carol obtained her BA Sc at and Business Certificate at Ryerson University, and completed her dietetic internship at London Health Sciences. In addition to her role as President of Seasons Care she also serves as the Chair of the Board of Directors Dietitians of Canada.
