From Admission to Discharge: Improving Resident Transitions

Transitions into and out of the range of care options for older adults are among the most stressful and challenging periods in the care experience for residents as well as for retirement and long-term care home operators.

Join us for our new 3-part webinar series that explores key care transitions and offers practical strategies to support residents, families, and staff. The series covers planning, move in, and discharges/evictions, and gives attendees useful tools and insights to help make transitions smoother, more coordinated, and more person-centred.


Day 1
Wednesday, March 25, 2026
10:00 am – 12:00 pm EST

> Receive an overview of six clinical profiles developed through homecare assessment data for Ontario older adults living in the community based on their medical, functional, psychosocial and cognitive needs.
> Learn how to apply these profiles to support effective planning and informed decision-making at point-of-care, organizational and system-wide levels.
> Participate in a facilitated discussion and activity aimed at leveraging the profiles to develop long-term strategies for identifying community needs, shaping future service priorities and strengthening the entire continuum of care.


Day 2
Tuesday, March 31, 2026
10:00 am – 12:00 pm EST

> Enhance your understanding of resident/family transitions into long-term care through the real-world stories and insights of Family Councils Ontario as well as how shared experiences can strengthen relationships between families and staff.
> Learn how emotional, practical and relational challenges shape the first six weeks in long-term care for families and the care team.
> Find out about programs and resources to improve the move-in process and make transitions more person-centred such as a new caregiver navigation toolkit, strategies for integrating family councils, and peer-to-peer support.


Day 3
Tuesday, April 7, 2026
10:00 am – 12:00 pm EST

> Understand the legal processes involved when dealing with resident discharges from long-term care homes and evictions from retirement homes and how these processes differ in the two settings.
> Gain insight into best practices to use when handling resident discharges and evictions.
> Learn about an operator’s rights and options when faced with challenging situations such as safety issues, interference of residents with the rights of other residents and staff, inability to meet increasing care requirements and non-payment of fees.

Important Notice: Your registration fee entitles you to ONE telephone line/VOIP connection. Any additional connections from the same home or organization will incur an additional charge for the full registration fee.

To have multiple staff members participate, each staff member must either complete a separate registration form and pay the full fee or listen in from the same room (e.g., a conference room).

Cancellations: Cancellations will be accepted up to five working days prior to the date of the program less a 15% administration fee that will apply regardless of the status of payment. Registrants who fail to attend the program or cancel after the deadline date shall be liable for the full fee. There is no charge for delegate substitutions. The AdvantAge Ontario Education Department reserves the right to cancel or reschedule this program.
