Webinar 1 – Using Clinical Profiles to Inform Care Planning and Decision Making
> Receive an overview of six clinical profiles developed through homecare assessment data for Ontario older adults living in the community based on their medical, functional, psychosocial and cognitive needs.
> Learn how to apply these profiles to support effective planning and informed decision-making at point-of-care, organizational and system-wide levels.
> Participate in a facilitated discussion and activity aimed at leveraging the profiles to develop long-term strategies for identifying community needs, shaping future service priorities and strengthening the entire continuum of care.

Executive Director & Scientific Director, SE Research Centre
Dr. Justine Giosa is a research scientist and leader with 15 years of experience bridging the knowledge to practice gap in health services. In her role as Executive and Scientific Director, Justine leads the SE Research Centre as an embedded centre of excellence for research and evaluation at SE Health. Dr. Giosa directs a mission-driven, investigator-led, and scientifically autonomous research portfolio and supports a diverse team of scientists, fellows, trainees and staff to conduct high quality, transparent, meaningful and rigorous studies. Justine is also an Adjunct Assistant Professor in the School of Public Health Sciences at University of Waterloo.

Clinical Scientist, SE Research Centre
Dr. Margaret Saari is a Senior Research Scientist and nurse researcher specializing in applied health services research in home and community care. Her program of research focuses on strengthening integrated health and social care systems to better support people living and aging in the community, particularly those with complex needs. Grounded in principles of health equity, person centredness, and system integration, her work spans innovative care models, health workforce development, and the use of data to inform care planning and delivery at the point of care. Margaret holds a PhD in Nursing Health Systems, is an interRAI Fellow, and holds an adjunct appointment at the Lawrence S. Bloomberg Faculty of Nursing.

Webinar 2 – Making the Move: Promising Practices for the Transition to LTC
> Enhance your understanding of resident/family transitions into long-term care through the real-world stories and insights of Family Councils Ontario as well as how shared experiences can strengthen relationships between families and staff.
> Learn how emotional, practical and relational challenges shape the first six weeks in long-term care for families and the care team.
> Find out about programs and resources to improve the move-in process and make transitions more person-centred such as a new caregiver navigation toolkit, strategies for integrating family councils, and peer-to-peer support.

Liane is passionate about making a difference and helping others, but most particularly those living, working and family members in LTC. She has 20 years of experience of working in a LTC home as a Resident and Family Services Coordinator, helping support residents and families as they transitioned into LTC. Part of her role was being the Staff Liaison for the home’s Family Council. Her years of experience helped her to develop great skills in communicating with and supporting families, residents and staff in LTC. Liane holds a background in Social Work. Through the years she has gained knowledge, experience and training in Palliative Care, Customer Service and Person Centred Care Approach. Throughout her career in LTC she has been involved in Strategic Planning, Goal Planning, Accreditation and Ministry Compliance. In her personal life, Liane loves to spend time with her family creating memories. She enjoys listening to music and watching home improvement shows, providing her husband of 31 years with a lengthy to do list.

Education Manager, Family Councils Ontario
Cathleen brings her passion to support families and advocate for inclusion, drawing inspiration from her work as a Recreation Professional in Long-Term Care.Cathleen holds an Honours Bachelor of Human Kinetics from the University of Windsor and a Master of Arts in Health & Physical Education from Brock University. While at Brock University, she completed the Instructional Skills Workshop (ISW) and earned her Graduate Teaching Assistant Practicum Certificate from the Centre for Teaching, Learning and Educational Technology. She is an experienced instructor, facilitating courses in Physical Education, Recreation and Gerontology at Brock University, the University of Alberta, Seneca College and Durham College.Prior to commencing her new role with Family Councils Ontario, Cathleen was the Work package 1 Coordinator for AGEWELL based out of the Toronto Rehabilitation Institute and a Professor in the Activation Coordination in Gerontology Program at Durham College.In her spare time she enjoys swimming, singing, playing her Alto Saxophone.

Webinar 3 – LTC Resident Discharges and Retirement Home Evictions: Legalities, Differences and Best Practices
> Understand the legal processes involved when dealing with resident discharges from long-term care homes and evictions from retirement homes and how these processes differ in the two settings.
> Gain insight into best practices to use when handling resident discharges and evictions.
> Learn about an operator’s rights and options when faced with challenging situations such as safety issues, interference of residents with the rights of other residents and staff, inability to meet increasing care requirements and non-payment of fees.
