Receive an overview of six clinical profiles developed through homecare assessment data for Ontario older adults living in the community based on their medical, functional, psychosocial and cognitive needs.
Learn how to apply these profiles to support effective planning and informed decision-making at point-of-care, organizational and system-wide levels.
Participate in a facilitated discussion and activity aimed at leveraging the profiles to develop long-term strategies for identifying community needs, shaping future service priorities and strengthening the entire continuum of care.

Executive Director & Scientific Director, SE Research Centre
Dr. Justine Giosa is a research scientist and leader with 15 years of experience bridging the knowledge to practice gap in health services. In her role as Executive and Scientific Director, Justine leads the SE Research Centre as an embedded centre of excellence for research and evaluation at SE Health. Dr. Giosa directs a mission-driven, investigator-led, and scientifically autonomous research portfolio and supports a diverse team of scientists, fellows, trainees and staff to conduct high quality, transparent, meaningful and rigorous studies. Justine is also an Adjunct Assistant Professor in the School of Public Health Sciences at University of Waterloo.

Clinical Scientist, SE Research Centre
Dr. Margaret Saari is a Senior Research Scientist and nurse researcher specializing in applied health services research in home and community care. Her program of research focuses on strengthening integrated health and social care systems to better support people living and aging in the community, particularly those with complex needs. Grounded in principles of health equity, person centredness, and system integration, her work spans innovative care models, health workforce development, and the use of data to inform care planning and delivery at the point of care. Margaret holds a PhD in Nursing Health Systems, is an interRAI Fellow, and holds an adjunct appointment at the Lawrence S. Bloomberg Faculty of Nursing.

Cancellations: Cancellations will be accepted up to five working days prior to the date of the program less a 15% administration fee that will apply regardless of the status of payment. Registrants who fail to attend the program or cancel after the deadline date shall be liable for the full fee. There is no charge for delegate substitutions. The AdvantAge Ontario Education Department reserves the right to cancel or reschedule this program.
